Referrals
Patients can be referred by their physicians or can themselves call up for an appointment if they have a referral from a doctor. For Referrals/Appointments call 514-340-8222 ext 8248. Patients must provide a brief updated medical summary and relevant medical reports including pathology, radiology or X-ray and operative reports. These can be sent by fax to  514-340-8302.

The Health Services Team
Physicians
The Hematologists/Oncologists are the physicians who determine the type of treatment you may need to follow. They see patients by appointment in the Hematology/Oncology Clinic between Monday and Friday. 

Primary Hematology/Oncology Nurses

Each new patient is assigned to a primary nurse. She is in charge of your case and plans the care for you (along with you and your significant others), in collaboration with various health professionals in the hospital, as well as others in the community. She provides you with teaching, counseling, and the support needed throughout your experience with cancer. Some specific examples of her roles are:

  • Evaluation of the impact of the illness on you and your family. 
  • Explanations and information related to chemotherapy and side effects as well as the management of symptoms related to the illness or medications (nausea, vomiting, fever, pain, constipation, diarrhea etc.)
  • Administration of chemotherapy and medications. 
  • Follow-ups on how you are coping, physically as well as emotionally with the treatment and the illness. 
  • Consultations with/referrals to other health care professions to respond to your various needs (dietitian, Hope & Cope, social worker, psychologist, psychiatrist, supportive care team, CLSC, etc.).

Bone Marrow Transplant (B.M.T.) Coordinator

The Bone Marrow Transplant (B.M.T) coordinator is a specialized nurse who coordinates all stem cell transplantations. She is in charge of each case and plans the transplantation in collaboration with a physician. She provides you with teaching, counseling and support needed throughout your experience. She is also responsible for the organization of various testing needed before and after the transplantation.

Some specific examples of her role are:

  • Evaluation of the impact of the transplantation on you and your family.
  • Explanation and information related to the transplantation and its side effects as well as management of symptoms (nausea, vomiting, fever, diarrhea, Graft-versus-host disease (GVHD), fatigue etc.). 
  • Coordination of various treatments with different departments to ensure proper timing of transplantation (oncology clinic, apheresis clinic, Hema-Quebec, inpatient department-7NW). 
  • Scheduling of transplantation, whether it be out-patient or in-patient. 
  • Follow-ups on how you are coping physically and emotionally with the transplantation. 
  • Consultations with other health care professionals to respond to your various needs (dietitian, Hope & Cope, social worker, psychologist, psychiatrist, supportive care team, CLSC etc.). 
  • Resource nurse. 

Dietitian

For patients who are actively undergoing chemotherapy treatments and are experiencing side effects, please contact your primary nurse who will arrange for a referral to the dietitian.Well-nourished patients tolerate cancer treatment better and seem to recover more quickly from treatment side effects.

The dietitian will provide individual counseling to:

  • Assess the dietary needs of patients.
  • Help patients to maintain good nutritional status and weight control.

Supportive Care Team (S.C.T.)

The Supportive Care Team specializes in pain and symptom control (nausea, vomiting, constipation, etc.) and emotional support for patients and families affected by a diagnosis of cancer. The S.C.T. keeps contact with patients by phone when they are at home, and sees patients in the Hematology/Oncology clinic and in the hospital. The team is composed of a clinical nurse consultant, family physicians, a liaison social worker, representatives from Hope & Cope, Hematology/Oncology nursing, psychology and psychiatry, the pharmacy and the Victorian Order of Nurses (V.O.N.). Your primary nurse can refer you to this team.

Psychosocial Support
Social Worker

The social worker’s role is geared toward enhancing the strengths of individuals or family members as they cope with the stresses created by cancer. Through counseling and linking with both hospital and a wide range of community resources, support is provided to patients and their families affected by the diagnosis of cancer. Psychologist/Psychiatrist If you are having particular difficulties adapting to the cancer diagnosis and/or treatment, the services of a psychologist and psychiatrist are available to you. Your nurse, doctor or social worker can make the necessary referrals.

Hope & Cope

The staff of Hope & Cope along with over 400 volunteers, provide a comprehensive range of free services to respond to the emotional and practical needs of men and women who are coping with the stress of a cancer diagnosis and its treatment. 

Pharmacy

The pharmacists provide patient counseling on questions about drugs such as:

  • Chemotherapy
  • Side effects 
  • Drug interactions 
  • Pain management and symptom control 
  • Pharmaceutical care

Receptionists

The role of the receptionists includes making additional appointments, booking tests such as x-rays and scans, and arranging follow-up visits. You must check in with them prior to any appointment that you have at the clinic. They do not give out any medical results.If you are having any medical problems, do not consult the receptionist but ask to speak to your nurse.

Some patients may be participating in a clinical trial which is coordinated by a Clinical Research Associate. The Clinical Research Associate directs you with respect to follow up appointments, scheduling of tests etc.
