Clinical Access Service

Mission

The Jewish General Hospital (JGH) Clinical Access Service is a new organizational model established to help patients with sub-acute or semi-urgent clinical conditions. Using a dedicated point of entry, patients gain rapid access to JGH diagnostic testing, Specialist consultation, and treatments.

Patients are referred to the Clinical Access Service by Community Family Doctors and Nurse Practitioners from within the CIUSSS West-Central Montreal territory.

Referred patient must meet general Clinical Access Service eligibility criteria, and inclusion and exclusion criteria for each clinical condition.

To refer patients, see the Referral Guide for Community Family Doctors and Nurse Practitioners.

Algorithms and referral documents for each clinical condition were developed in collaboration with JGH Specialists. These documents direct the Clinical Access nursing team in the provision of diagnostic testing, specialty consultation, and treatments.

Notice to Patients

During your in-person appointment at the Access Clinic, delays may occur, particularly if an imaging exam is required. We recommend planning for a full day to allow for the completion of your evaluation and tests. We will make every effort to provide prompt and efficient service in order to minimize your waiting time.

In preparing for your appointment and to help prevent delays, please bring these items with you:

  • Your health insurance card (RAMQ)
  • Your valid Jewish General Hospital card
  • An up-to-date list of your medications (available for free from your local pharmacy)

Please ask your local pharmacy to fax a copy of your current medication list to 514‑340‑8616.

Please note that if any of this information is missing, you may experience delays at your appointment.
