Decision Tools

Disclaimer: This information should not be considered as medical advice. It is not to be used in place of a visit with a doctor, nurse or other healthcare professional. If you have questions about your individual medical situation, please consult with your healthcare professional.
Decision tools are designed to help you make a difficult decision. Some of them are interactive and can be filled out online, and others can be printed and filled out with a pen or pencil. The idea is for you to see what the pros and cons are and to take some time to think about what matters to you. Once you have filled one out you can bring it to your next appointment and talk to your doctor about what you have decided or about any questions or concerns you might have.
