When will my first time appointment be?
Upon receipt of your referral, you will be contacted by the Medical Secretary Vandella Anderson as soon as our team can provide you with appropriate high quality care. Referrals can be sent by fax to (514) 340-8705.

When will my surgery be?
Usually one week prior to your surgery, Lianna Mantley, the Administrative Agent of the clinic, will call you to confirm the details of your surgery, including the date and time. Please understand that due to healthcare resource constraints, wait times may vary. If there is a change to your surgical date, Lianna will call you to inform you of this change.

When will my first post-op visit be?
Upon your discharge from the hospital following your surgery, you will obtain an appointment for your first post-op visit. If your appointment includes pathology results, please contact the Medical Secretary Vandella Anderson one day before to check the availability of the results at (514) 340-8222 extension 23114 option 1.

If I have a form to be completed by my physician, who do I give this form to?
Please contact the Administrative Agent Lianna Mantley at (514) 340-8222 extension 23114 option 4. You can also fax the form to her at (514) 340-8619 or you can drop the form off in person in E-728. Once the form has been completed, Lianna will contact you to inform you that it is ready to be picked up. Please also note that fees may apply for the completion of forms.

How can I get a copy of my chart?
Please contact the Administrative Agent Lianna Mantley at (514) 340-8222 extension 23114 option 4. Depending on the document(s) needed, she will either give you a copy of the report(s), or she will refer you to the Medical Records Department.

I just had my test done. When and how do I find out about my test results?
In order to protect patient privacy, test results are not given out over the phone. They are only given out by your physician at your follow-up appointment. If you do not have an appointment, you can call the Medical Secretary Vandella Anderson to arrange one at (514) 340-8222 extension 23114 option 1.

When will my follow-up appointment be?
Based on your specific needs, the team will determine your next appointment. Medical Secretary Rania Derwiche will contact you following your visit to schedule the future appointment.
