New patients - getting an appointment

Dr. Antoniou

To be placed on a waiting list for Dr. Antoniou you must bring your x-rays on a CD,
written reports and the referral from your family doctor to our office in room E-003 from Monday to Thursday between 9-5.

Please note that Dr. Antoniou only looks after hip or knee problems and the wait time of 6-12 months is based on what he sees on the x-rays.

Dr. Bergeron

Please fax your MRI report along with the referral from your doctor to 514-340-7595.

You will be called for an appointment.

Dr. Chaytor

In order to obtain an appointment you must have a referral and x-rays.

Please fax your documents to 514-340-7595 or bring the information to room E003.
Once your request has been accepted by the surgeon, wait times will vary according to priority.

Dr. Huk

In order to get an appointment, you need to fax at 514-340-7595 a referral from your doctor along with x-rays.
You can also bring the documents at our office in room E-003.

Please note that the wait time for an appointment may be up to 6 months.

Dr. Jarzem

For an appointment, you will need a referal from your doctor, an MRI report and the spine questionnaire. You can find the questionnaire  here.
If your MRI was not done at the Jewish General Hospital you will need to bring the CD the day of your appointment.

Our wait time is presently from 6-8 months or according to priority.

Dr. Rosen

Please fax consult and report of x-ray to 514-738-8752. You will be called with an appointment.

Dr. Volesky

For an appointment for a foot or ankle problem, you must have a referral and xrays.
For a ligament tear in the knee, you must have a referral and an MRI.

Please fax your documents to 514-340-7595 or bring the information to room E003.
Once your request has been accepted by the surgeon, wait times will vary according to priority.

Dr. Zukor

Please bring/mail us your most current x-rays on a disc along with the referral from your doctor.

We will call you with an appointment.
Please note that wait time may vary between 6-8 months for a consultation.
